The undersigned parent or legal guardian and player hereby acknowledge that participating in the Tamir Goodman Basketball Camp/Clinic carries with it the potential risk of injury, and as such the undersigned hereby assumes the risk of such possible injury. I assume financial and legal responsibility for any injury or injuries suffered during participation in the above mentioned sports camp/clinic. I am aware of the risks and assume the responsibilities associated with participation in the camp/clinic.. Recognizing the possibility of physical injury, I hereby release, discharge and/or indemnify Tamir Goodman Basketball Camp,, its directors, coaches, sponsors, employees and associated personnel, including the facilities utilized for the Programs, against any claim, loss, damage or other disability. Tamir Goodman Basketball Camp and its employees or agents are not responsible for accidents and medical and dental expenses incurred as a result of participation in this program. My child is covered by family/personal insurance, is under the care of a doctor and is deemed to be in good health and able to participate in the physical activity of a rigorous sports program. I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT & FULLY UNDERSTAND ITS TERMS. Please also send us a note from your doctor that your child is healthy to train. Please also purchase a travel/health insurance policy.*

Tamir Goodman Basketball Camp Indemnification: The undersigned parent and/or legal guardian does hereby represent that he/she is, in fact, acting in such capacity and is fully responsible. He/she agrees to waive, hold harmless, release and indemnify each and all of the parties associated with the Tamir Goodman Basketball Camp from all liability, loss, cost, claim or damage whatsoever arising from both ordinary and gross negligence.*
